We are serious about this. Your name and the information you send stay with Caydra LLC. We use them only to communicate with you.

What this means, in plain words

We do not use your name, address, phone, email, license, NPI, or written opinions for anything except verifying that you are a licensed clinician and communicating with you about Caydra. We do not publish those details. We do not sell them. We do not use them in advertising, testimonials, or mailing lists for other products.

What we collect

Registration (one form): full name, work email, phone, city and state or province, role, and NPI, license number, or faculty appointment.

Opinion form: full name, work email, phone, address, and the changes you say are needed.

Why we collect it

To confirm you are a real licensed clinician (or qualifying faculty), to send one month of free Caydra, and to hear what you think should change.

What we do not do

We do not publish your name. We do not sell your information. We do not use it in ads. We do not share it with marketers. We do not use it as a public case study without a separate written yes from you.

Caydra LLC, at michael.p@caydra.io. Forms are delivered through FormSubmit to that mailbox.

If you want it removed

Email michael.p@caydra.io and ask us to delete your details. We will.
